Nancy Ann Cox, called Ann, was born in Virginia on 17 October 1757. She was one of the twelve children of Isaac Cox and Susannah Tomlinson, who lived on the South Branch of the Potomac in Hampshire County. Her brother David was the father of Margaret Cox, who later married her son Friend. On 21 March 1774, at sixteen, she married William McMahon in Hampshire County. Their son William Friend was born in Ohio County on 25 October 1776, and a 1948 history of the Coxes names eight children of the marriage: Friend, Richard, Susannah, James, John, Joseph, Polly, who married Joseph Agnew, and Nancy, who married John Bukey.

Her husband was killed at Fort Recovery in the Ohio country on 30 June 1794, commanding the escort of an army supply train. On 15 October 1797 she married Edward Worrell in Brooke County, the county cut from Ohio County that year. A descendant who read the Brooke County deeds found land she had from her first husband, sold by her and Edward, with William named as her deceased husband and some of the deeds naming his children. Her sons also brought suit as William's heirs at law in the county court.

William left a will and named her his executrix. In January 1795, acting as "Ann McMahan Executrix of William McMahan," she took a lease of town property from James Marshall in the addition to Charlestown, the town on the Ohio now called Wellsburg, and bought seven more lots there from Absalom Wells and his wife Michal.

Her son Friend went down the Ohio to Nelson County, Kentucky, and married his cousin Peggy Cox there in 1798. Ann stayed in Brooke County. She died at Wellsburg on 17 January 1818, aged 60. Where she is buried is not known.

Her father died in 1783 and his estate was settled in Nelson County, Kentucky, in 1800. Of the nine heirs then living, she and her brother George were the two still on the upper Ohio; her mother and the rest had gone down the river to Kentucky. Three of her brothers had been killed on the frontier, James as a young man in Hampshire County, Joseph on the Ohio in 1780 and Col. Isaac Cox in Kentucky in 1788.
